Gabriela Gonzalez Resource Management Flight DAVIS-MONTHAN AIR FORCE BASE, Ariz. -- The Third Party Collection program is a congressionally mandated initiative requiring the billing of private health insurance companies when possible. All retired military personnel and all dependents (whether the sponsor is retired or not) must complete a DD Form 2569, Third Party Collection Program Insurance Information form annually or when their insurance information changes. The Department of Defense requires this form be completed even if Medicare or TRICARE are your only insurance providers. The forms are available at all medical clinic reception areas as well as at the Base Exchange Pharmacy. Completing the 2569 is an easy process and only takes a couple minutes to fill out. Once you completed your TPC form you will receive a Verification Card, make sure you carry this card with you at each visit. Although the requirements of this program may seem like another administrative headache, it is important to remember that third party billing is required by law and that collections do come back to our facility.
